This position provides targeted case management activities primarily for individuals diagnosed with serious mental illnesses. Works as part of the team to assure adults are linked with necessary services, supports and resources and ensures supports are coordinated among providers as needed to help individuals served live and function optimally in the community.
Case Manager I: Bachelor's Degree in special education, social work, psychology or other acceptable human services related field or an unrelated Bachelor’s Degree that includes at least 15 semester credits in a human services field. Clinical experience working with individuals with mental health diagnoses is helpful.
Case Manager II: Bachelor's Degree in special education, social work, psychology or other acceptable human services related field with at least one year of applicable related clinical experience working with individuals with mental health needs and/or developmental disabilities or an unrelated Bachelor’s Degree that includes at least 15 semester credits in a human services field and at least three years of applicable clinical experience. Virginia DMAS’ QMHP and/or QIDP status is required. QSAP status or eligibility is helpful.
Either position requires: CPR and FA (training provided), possession of a valid driver’s license (Virginia, unless otherwise approved by agency) and an acceptable driving record must be maintained throughout employment;

$32,940 annually for Case Manager II
