Position open for Community Support Professional to work with for individuals with intellectual disabilities and/or co-occurring mental health diagnoses participating in a community based day support program. Works as part of the team to ensure all Medicaid and DBHDS Licensure standards are maintained. Works with the team to develop activities which promote meaningful community involvement and writes Person Centered Plans. Completes quarterly and annual reviews, ensures program’s daily documentation is complete and accurate, and coordinates supports with other providers. Some evening and weekend work will be required.
Bachelor’s degree in Human Services related field is required. Must meet the knowledge, skills and abilities for Case Manager as set forth by the Virginia Medicaid. Prefer applicants with at least one year of experience working with individuals with mental intellectual disabilities but will consider individuals who have at least 9 months of applicable experience. Certification in CPR and First Aid are required (training provided). Must have an acceptable driving record.
