Provides case management services for children and adolescents identified as eligible for services due to developmental delay, diagnosis of Autism Spectrum Disorders and/or Developmental Disabilities. Coordinates services and/or resources needed by the child to be maintained in and/or return to community living.
Graduation from an accredited college or university with a Bachelor's degree in Social Work or human services related field is required, with at least one year of experience in a setting working with children with needs in the areas of mental health or developmental disability preferred. Experience in Behavioral Management techniques preferred. Demonstrated knowledge of case management/social work practices and resources available to help consumers in the community. Excellent communication skills required. Valid Virginia driver's license and safe driving record required.
