  • Overview

    Special Deliveries is a program that works with prenatal women and families with children under age 5 by using a home visitation model of practice.  This Case Manager will assist in providing individualized services for parents and children who may experience mental health concerns, substance use issues, and/or developmental delay.  Position provides supportive counseling and case management services on a weekly basis.  Coordinates services and/or resources needed by the client to support their basic needs and enable them to remain and function optimally in the community.  This position will also be responsible for completing clinical documentation and wrap around case management services such as completion of referrals, collaboration with community stakeholders, parenting skills, and any other appropriate client needs that may arise.  Position promotes communication among all parties providing services to the client and participates in treatment planning meetings, as needed, to ensure continuity of care. This position will be located primarily in Radford. 

     

    Cell phone, laptop, weekly supervision and mileage reimbursement provided.

    Qualifications

    Graduation from an accredited college or university with a Bachelor’s degree in Social Work or human services related field, with at least one year of experience in a setting working with children or adults with needs in the areas of mental health, substance use disorders, developmental delay, or developmental disability is required. An appropriate applicant will demonstrate knowledge of case management/social work practices and resources available to help consumers in the community. Valid Virginia driver’s license with a safe driving record required.
